Subsidized Guardianship Program

Subsidized Guardianship Program provides monthly board and care payments and fully paid HUSKY Health medical insurance to relatives or family friends caring for children placed in their care.

How to apply

• A relative caregiver initiates a request for a guardianship subsidy by applying to the child's DCF caseworker (available after the child has been in DCF care/certified relative care at least 17-18 months). The caseworker provides the application form (DCF-2159) and assists with completion.
