Money Follows the Person

Money Follows the Person is a federal grant program that helps members aged 18 and older with disabilities transition from institutions to community living, providing demonstration services for 365 days after discharge and supplemental services including food assistance, environmental adaptations, and rental assistance.

• Applicant age: at least 18

How to apply

• Referral to a Local Contact Agency (LCA) for Options Counseling, or contact the Member Contact Center; a family member, friend, advocate, or provider can also make a referral on the member's behalf
