Elderly, Blind, and Disabled Waiver

Elderly, Blind, and Disabled Waiver provides Colorado Medicaid beneficiaries with disabilities home and community-based services including respite, adult day care, and home modifications.

Eligibility

• Care recipient must have a qualifying disability
• Care recipient age: at least 18

How to apply

• If you are already enrolled in Health First Colorado, contact your local Case Management Agency to start the waiver application process. If NOT a Health First Colorado member, contact your local CMA, or apply online at co.gov/PEAK.
