Program of All-Inclusive Care for the Elderly (PACE)

The Program of All-Inclusive Care for the Elderly (PACE) model of care provides a comprehensive medical/social service delivery system using an interdisciplinary team approach in a PACE Center that provides and coordinates all needed preventive, primary, acute and long-term care services.

Eligibility

  • Care recipient must have a qualifying disability
  • Applicant age: at least 55
  • Care recipient age: at least 55

How to apply

  • Find your local PACE organization by county using the Service Area Counties and Zip Codes list and PACE organization Contact Information; you must reside in a PACE service area and be determined eligible at the nursing home level of care to enroll.
