Arizona Self-Directed Attendant Care — SDAC and Agency with Choice (ALTCS Managed Care)¶
Arizona Long Term Care System (ALTCS) is a managed care 1115 waiver program providing self-directed attendant care through two integrated options within ALTCS managed care plans: Self-Directed Attendant Care (SDAC), where participants hire and manage their own attendants including family members, and Agency with Choice (AWC), a co-employment model where an agency handles payroll/HR while the participant directs care. Both SDAC and AWC are offered through ALTCS managed care contractors (DES/DDD, CMDP, and commercial plans). Participants can hire family members including spouses as paid caregivers under SDAC. ALTCS is an entitlement program within AHCCCS (no formal waitlist), though enrollment processing can be lengthy.
