Alaska Community First Choice (CFC) — 1915(k) State Plan HCBS

Alaska Community First Choice (CFC) is a distinct 1915(k) state-plan Medicaid benefit providing home and community-based services for individuals meeting an institutional level of care. CFC emphasizes participant autonomy and includes personal care assistance, Personal Emergency Response Systems (PERS), case management, chore services, and skill building. CFC is separate from Alaska Personal Care Services (PCS), which is a state-plan personal care benefit for individuals not requiring institutional level of care. Family members can be hired as paid attendants under the Consumer Directed Personal Care Services Program (CDPCS) available under both programs.

Requires institutional level of care (distinct from Alaska PCS which serves individuals below that threshold). FY2026 rates: $9.01 per 15-minute unit for personal care; $11.66 for skill building; $9.43 for chore services. Consumer Directed Personal Care Services (CDPCS) option allows hiring family members.
