VA Home-Based Primary Care (HBPC)

Interdisciplinary VA primary care team — physician-led, including nursing, social work, mental health, rehabilitation, pharmacy, and dietitian — that delivers comprehensive primary care in the veteran's home. Designed for veterans too complex or frail for routine clinic visits. Includes caregiver education and support, directly reducing family caregiver transport and coordination burden.

Comprehensive primary care delivered in the veteran's home by an interdisciplinary team: physician oversight, registered nursing visits, social work assessment and care coordination, mental health support, physical/occupational therapy, pharmacy management, and nutritional counseling. For family caregivers: (1) eliminates the need to transport a complex/frail veteran to clinic appointments; (2) social worker connects family to VA caregiver services (PCAFC, PGCSS, respite) and community resources; (3) team educates family caregivers on safe patient handling, medication management, and symptom recognition. Available at most VA medical centers (~130 HBPC programs nationwide as of 2024).
