Money Follows the Person (MFP) Rebalancing Demonstration¶
Federal demonstration grant that funds enhanced Medicaid match for transitioning individuals from nursing facilities and other institutions back to home and community settings. Provides transition coordination, home setup, and caregiver supports during the 365-day post-transition period. Active in 45 states, DC, and 2 territories. The program is effectively sunsetting: new transitions ended December 31, 2025, with MFP-matched services available through December 31, 2026.
MFP covers: (1) Transition coordination services — helping families plan the move from institution to home, identify housing, arrange equipment; (2) Home setup costs — one-time expenses like first/last month rent, utility deposits, basic furnishings; (3) HCBS during the 365-day transition period — Medicaid-reimbursable services while the newly transitioned person establishes community living; (4) Caregiver supports — training, counseling, and respite for family caregivers participating in the transition. Sunsetting: Federal grantees could only enroll NEW individuals through December 31, 2025. States with active cases may continue MFP-enhanced-match services through December 31, 2026. Unused FY2023 federal award funds remain available through FY2027. Contact your state Medicaid office to confirm MFP status and whether referrals are still being accepted.
