Medicare GUIDE (Guiding an Improved Dementia Experience)

CMS Innovation Center demonstration model launched July 2024 providing comprehensive dementia care services for 8 years. Includes a dedicated care navigator, 24/7 support line, respite services (up to $2,500/year), caregiver training, and support services for unpaid caregivers. Designed for Medicare beneficiaries with dementia and their unpaid caregivers. 327 participating organizations across the nation in two tracks (Established Program and New Program). This is a CMS demonstration under Section 1115A -- availability depends on local participating organizations.

Comprehensive 8-year dementia care model providing: dedicated care navigator, 24/7 access to support line, respite services (up to $2,500/year per eligible patient), caregiver training and education, connections to community resources, and screening for health-related social needs.
