Louisiana Monitored In-Home Caregiving (MIHC) - Community Choices Waiver¶
Monitored In-Home Caregiving (MIHC) is a service under the Community Choices Waiver (CCW) where a principal caregiver lives with the participant and provides basic self-care and assistance with activities of daily living. The caregiver receives a daily stipend through a professional agency that provides oversight and training.
Application Process: 1. Contact Louisiana Options in Long Term-Care at 1-877-456-1146 to be screened and added to the Request for Services Registry (RFSR). 2. When a slot becomes available (on a first-come, first-served basis, unless priority criteria are met), a face-to-face assessment (MDS-HC) is conducted. 3. Financial and medical eligibility is determined, and a Plan of Care is developed with a Support Coordinator.
Providers: Participants can browse the Freedom of Choice (FOC) Provider Listings at https://ldh.la.gov/directory/foc. Providers must be licensed by the Health Standards Section with an MIHC module.
The daily rates are: Tier 1 (Level 1) at $59.60 per day and Tier 2 (Level 2) at $89.40 per day. The waiver has a total enrollment cap of 8,053 funded slots (as of Oct 2025).
