Indiana Pathways for Aging / Structured Family Caregiving

Indiana offers self-directed attendant care under the Pathways for Aging waiver, allowing Medicaid-eligible individuals with functional limitations to hire and manage their own caregivers. The state also provides Structured Family Caregiving (SFC) administered through Careforth for live-in caregivers. Indiana was a Cash & Counseling demonstration state, giving it a long history with consumer-directed care models.
